The description seems to best fit Velvet. Yes, medications often treat
against a number of other diseases, but may not necessarily treat against an
entirely different pathogen. It's encouraging that this Gourami is more
active. While anything may still happen, at least it's a change in the right
direction, if this behavior is actually as it appears. Notice if the fishes
are eating. If the Gourami eats, it usally indicates it can't be feeling too
bad.
Not all fishes get certain diseases just because they're in the same tank.
Gourami's are among those fishes which are particularly susceptible to
Velvet, and are more prone to getting it when others may not.
Many hobbyist treating Ich with the heat treatment maintain the temperature
at 30 C or more for periods of up to 10 days. While a higher temperature
will boost their immune system initially, prolonged high temperatures such as
at 30 o will tend to weaken some (depending on the species) over time as it
causes stress on them when well above their natural range. Added
circulation helps in easing this stress as it ensures the most oxygen in the water
for the temperature it's being held at. I've never seen a fish die of high
temperatures such as those I mentioned, though.
I wouldn't be too concerned at this time about non-life-threatening issues
such as split ventral fins ( feelers). They can always be addressed after
the main disease is cured. Continue to keep a watchful eye on that Pleco
when you re-dose.
