How big are these worms? Can you get a picture of them on a piece of white paper or white paper towel?
For worms to have eyes tells me they probably aren't worms but better chance they are larvae of something... could be a lot of different types of insect... a pic would really help a lot, and something in the photo to show size range of the worms.
Also, can you remove any from the water and see what happens if you put them into something like a little bit of dirt? Do they crawl into & live in the dirt or die being out of water?
There are many things that can come in on plants, driftwood (especially if the wood is not store bought but rather something found outside in the yard or at a beach), and various other things that go into an aquarium.
I'll do what I can to help ID them if you can get some photos and info about them... and where in your tank you found them (in the substrate, floating in the water column, in the filter media, stuck to plants or plant roots, etc.).
