I haven't kept up with this thread in its entirety, sorry... so I don't know what other fish are in the tank? If you can treat in quarantine, the most effective med for treating fungal infections in rainbows is a medication called Spectrogram that isn't made anymore. The good news is that I know the ingredients and can even offer a reliable place online to get them. Kanamycin and Nitrofurazone are the 2 ingredients/med you need. Dose both meds according to directions, at same time. These 2 are safe to mix.
This is not a safe combination for all fish, so please be very careful what species you expose to this med combo.
